On March 27, 1977, two Boeing 747 aircraft collided on the runway at Los Rodeos Airport on the island of Tenerife, killing 583 people. It remains the deadliest accident in aviation history. The KLM captain, one of the most experienced pilots in the airline's fleet, began his takeoff roll without clearance. He did not lack information. The co-pilot had expressed hesitation. The tower's communications contained cues that the runway was not clear. Fog prevented visual confirmation. Every piece of evidence necessary to avert the disaster was available inside the cockpit. None of it penetrated the interpretation the captain had already committed to: the runway is clear, we are cleared for takeoff, the sequence is proceeding as expected. Weick's 1990 analysis, "The Vulnerable System," transformed Tenerife from an aviation accident into the canonical case study of organizational sensemaking failure — not a story about a bad decision, but a story about an interpretation that committed too early to coherence and then filtered every contradictory cue through its own assumptions.

The context was chaotic. Both aircraft had been diverted from Gran Canaria after a bomb threat. Los Rodeos was overcrowded, with aircraft parked on the taxiway. Fog rolled in, reducing visibility. Communications were garbled by simultaneous transmissions. The KLM captain was under schedule pressure — crew duty hours were approaching limits that would require an overnight delay. Every element of the situation pressed toward the interpretation the captain had constructed: we are ready, we are cleared, we are proceeding.

The co-pilot's hesitation was documented. He questioned whether they had received takeoff clearance. The captain's response — essentially, of course we have — reframed the question as deference rather than challenge. The tower's ambiguous phrasing ("OK... stand by for takeoff") was interpreted by the KLM crew as confirmation rather than as caution. The Pan Am aircraft on the runway, which the KLM crew could not see due to fog, was invisible not only optically but interpretively: nothing in the captain's framework allowed for another aircraft to be where he was about to go.

Weick's analysis refused the easy frame — pilot error, schedule pressure, bad luck — and insisted on the structural reading. The captain was not stupid or reckless. His interpretation was plausible, coherent, identity-confirming, and actionable. The interpretation was wrong. But the organizational mechanisms that would have tested the interpretation against the contradictory cues — the co-pilot's authority to challenge, the redundant communication channels, the deference to expertise regardless of rank — had been eroded by the interpretation's own momentum.

The paper established a template that Weick would use across his subsequent work. Organizational disasters are not produced by missing information or by stupid people. They are produced by interpretations that achieve premature closure and then, by filtering cues through their own assumptions, make themselves self-sealing. The disaster is the moment when the interpretation can no longer be maintained in the face of reality. The events leading to the disaster are the period during which the interpretation was maintained while reality was demonstrating its inadequacy.

The parallel to AI-era organizational decision-making is uncomfortable. When plausible interpretations arrive at AI speed, the organizational mechanisms that would have tested them — the pause for deliberation, the authorized challenge, the cross-functional review — are structurally disadvantaged. The interpretation gets enacted before the testing happens. The testing, when it happens, is against an enacted reality that already reflects the interpretation.

Origin

Weick's "The Vulnerable System: An Analysis of the Tenerife Air Disaster" appeared in the Journal of Management in 1990, drawing on the official accident investigation reports and subsequent analyses. The paper became, alongside the Mann Gulch study, one of Weick's two most influential applied analyses.

The template generalizes. Every organizational disaster Weick studied exhibited the same structure: plausible interpretation, premature closure, systematic filtering of disconfirming cues.
