"Ironies of Automation" is Lisanne Bainbridge's landmark 1983 paper in Automatica that identified the structural problem built into any design philosophy that automates as much of a task as possible while leaving humans as backup operators. As automation takes over more of the task, the human operator's remaining responsibilities — supervising the automation, handling edge cases — become harder, not easier. Humans are poor monitors of reliable systems; they lose, through disuse, the skills that would let them recover when the automation fails; and the cases left to them are by definition the hardest. Every subsequent framework for automation design — airline cockpit human factors, autonomous-vehicle handoffs, medical-device alarms, AI-assisted workflows — sits downstream of Bainbridge's insights.
Bainbridge's paper is the operational counterpart to fictional worries about automation dependence. If Asimov's Solarians are the speculation, Bainbridge is the instrument panel. Her paper is short (seven pages), not mathematical, and named one of the most cited documents in human-factors engineering.
The specific aviation examples Bainbridge drew on in 1983 — autopilot-induced complacency, training loss among pilots who seldom hand-fly — became operational concerns in the subsequent decades. The 2009 Air France 447 crash, in which pilots who had rarely flown by hand were unable to recover from an unusual-attitude event the autopilot had handed off to them, is a textbook Bainbridge outcome. The aviation-safety community responded with mandatory hand-flying practice and explicit supervisory-skill training.
The AI era has brought Bainbridge's concerns to workflows she did not originally study. A lawyer using an AI assistant to draft briefs may lose the close-reading skill that catches AI hallucinations. A radiologist using AI to pre-read scans may lose sensitivity on the cases where the AI is wrong. A software engineer using an AI to write code may lose the debugging intuition that catches the subtle error the AI introduced. In each case, the human is asked to supervise reliable-but-imperfect automation with skills that the automation's reliability is eroding.

Bainbridge was a cognitive psychologist working in process-control industries (chemical plants, power generation); she later worked on aviation human factors and is counted among the founders of the cognitive-engineering field.
The supervisory irony. The operator is asked to monitor a system that works correctly 99.9% of the time — a task humans do poorly because vigilance degrades with time-on-watch and cues that don't appear.
The skill irony. The operator is expected to take over in crises, but their skill has atrophied because they never practice. Recovery requires exactly the skill that automation removed.
The specification irony. The designer must specify in advance which edge cases the automation cannot handle; by definition, the cases left to the human are the ones the designer could not anticipate or chose not to automate.
The trust irony. Too little trust in the automation, and the operator over-intervenes and undermines it; too much trust, and the operator under-intervenes and misses the failure.
Feedback design matters. The automation's behavior must be legible enough for the supervising human to diagnose problems, which is in tension with the automation being a black-box optimizer.
