The primary challenge to Vaughan’s framework is that it is too universal: if normalization of deviance is an emergent property of any complex organization operating under production pressure, how can any organization achieve genuine safety? Vaughan’s answer, implicit in her comparative studies of high-reliability organizations, is that the organizations that maintain safety against the mechanism are those that have institutionalized the redistribution of the asymmetry of burden—crew resource management in aviation, the stopping-the-line authority in Toyota manufacturing, the anonymous near-miss reporting systems in nuclear power—so that proceeding under uncertainty bears at least as much evidentiary burden as stopping. None of these solutions translates easily to AI-augmented knowledge work, where the production pressure is internal rather than external, the standards are implicit rather than codified, and the tool is specifically designed to make proceeding faster and stopping seem unnecessary. A second debate concerns whether the AI transition presents genuinely novel risks beyond Vaughan’s framework, because the normalization she documented operated against a background of standards that at least existed formally. In AI-augmented knowledge work, the erosion may be occurring to standards that were never written down: the professional norm that understanding is integral to competence has no specification document, and when it drifts, there is nothing to compare the practice against except the practice itself. The gap may therefore be unmeasurable rather than merely unrecognized—and an unmeasurable gap cannot trigger the institutional alarms that Vaughan’s framework requires to function.
