The system I was most proud of was the one most likely to kill us.

Not the AI. Not Claude Code. The management system. The org chart, the sprint cadence, the approval chains, the reporting structure I had spent years refining into something I considered elegant. It worked. It had worked for a long time. And the reason it was dangerous was precisely that it had worked, because working meant it had earned my trust, and trust meant I never questioned whether it was adequate for what was coming.
