Traditional development contained errors naturally through the slowness of implementation. The programmer caught errors as she wrote because the act of writing forced attention to each component individually. She noticed the incorrect assumption in line forty-seven because she had thought carefully about lines one through forty-six. AI-assisted development removes this natural containment. Code is produced faster than the user can evaluate it. An interpretation error in the authentication module propagates through session management, authorization logic, and user interface before anyone notices. By the time the error surfaces — weeks later, under conditions the user did not test — it has shaped dozens of downstream decisions, each internally consistent but founded on a false premise.
Cascading errors are the compound interest of the judgment gap. Each unevaluated assumption is a small debt. The debts accumulate silently, the interest compounds, and the total comes due at the worst possible moment — when the system is deployed, when users depend on it, when the cost of correction is orders of magnitude higher than the cost of prevention would have been.
The Orange Pill describes this dynamic through builders who moved fast with AI tools and discovered, weeks later, that their rapidly constructed systems contained embedded assumptions they had never examined. The problem was not carelessness. It was that the speed of production outpaced the speed of evaluation, and the system provided no mechanism for slowing production to match evaluation capacity or accelerating evaluation to match production speed.
Norman diagnosed this structurally as a feedback failure. In traditional systems, feedback on an action was immediate: press the button, see the result, know whether you succeeded. Feedback on AI-generated code is temporally displaced. The code works today. It may fail under load tomorrow. The security vulnerability may not surface for months. The architectural weakness may not become apparent until the system is extended in a direction the original design did not anticipate. The most consequential feedback arrives long after the decision to accept the output has been made, and the user cannot learn from feedback she does not receive until the damage is done.
The design response requires intervening at multiple timescales. At the individual interaction, confidence signals and interpretation previews should flag uncertainty before it propagates. At the workflow level, evaluation pacing mechanisms should slow production when the unevaluated debt grows too large, or flag accumulated assumptions before they cascade. At the long-term relationship level, the system should track patterns of acceptance versus modification and alert the user when her evaluation behavior is trending toward hazardous reliance.
The cascading error concept emerged from empirical observation of AI-assisted development failures in 2024–2025 and receives formal treatment in Chapter 4 of the Norman volume.
The pattern has structural affinities with Charles Perrow's normal accidents framework, where tight coupling and interactive complexity guarantee failure propagation. The Norman volume extends this analysis to the coupling between human and AI in high-velocity knowledge work.
