Marcello Massimini

Italian neurophysiologist whose collaboration with Tononi produced the Perturbational Complexity Index — the first empirically validated consciousness meter, transforming both clinical neurology and the measurement of inner experience.
Marcello Massimini is Professor of Physiology at the University of Milan and one of Tononi's principal collaborators. Trained in neurophysiology and transcranial magnetic stimulation, Massimini combined TMS with high-density EEG to develop the Perturbational Complexity Index — the clinical instrument that operationalizes IIT's predictions and distinguishes conscious from unconscious brain states with remarkable accuracy. His work has transformed the clinical assessment of disorders of consciousness, identifying awareness in patients previously diagnosed as vegetative. With Tononi, he authored Sizing Up Consciousness (2018), the definitive exposition of the theoretical and empirical framework behind the PCI.

Massimini's research program addresses a specific clinical problem: how to determine whether an unresponsive patient is conscious when behavioral assessment fails. His innovation was methodological — combining TMS (which perturbs the cortex directly) with high-density EEG (which measures the response with millisecond resolution and spatial detail) to probe the brain's causal structure without relying on behavioral output. The PCI compresses the spatiotemporal response into a
